Job Description

Join Superior HealthPlan as a Care Manager RN in the Foster Care division. This hybrid position, based in Texas, offers the opportunity to impact the lives of members by providing comprehensive medical and supportive services coordination.

Responsibilities

  • Assess and coordinate medical and supporting services for select members across the continuum of care.
  • Develop and implement individualized care plans to meet member's needs and desired outcomes.
  • Coordinate services between healthcare providers and non-medical staff to address complex medical, socio-economic, and community needs.
  • Provide patient and provider education, facilitate access to community-based services, and monitor referrals for overall care management.
  • Participate in integrated team care management rounds and identify risk management quality concerns.
  • Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems.
  • Review and approve assessments and care plans, coordinate service request authorizations, and facilitate member linkages to MCO-based services.
  • Assist members and/or agency staff with transportation to healthcare appointments and communicate health indicators to contribute to successful community living.

Requirements

  • Graduate from an Accredited School of Nursing, with a preference for a Bachelor’s degree in Nursing.
  • 2+ years of clinical nursing experience, 1+ years of case management experience in a managed care setting, and knowledge of utilization management principles and healthcare managed care.
  • Current state’s RN license.
