• Job Type: Full-time, Part-time, Per Diem
• Pay: $22.00 - $28.00 per hour
• Expected Hours: 20 – 40 per week
• Work Location: In person

Requirements

• HSD or GED required
• Current DE LPN license or compact state license

Duties and Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community
• Implement patient-centered care plans aligned with available resources
• Participate in discharge planning
