ChristianaCare HomeHealth is looking for dedicated Full Time, Part Time, and Per Diem Licensed Practical Nurses (LPNs) to join our team. As an LPN with ChristianaCare, you will provide skilled nursing care and treatment to geriatric patients in the community, participate in developing patient care plans, and contribute to a patient-centered approach.

Duties and Responsibilities

• Deliver skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement goal-oriented and patient-centered care plans.
• Participate in discharge planning for patients.

Requirements

• High School Diploma or GED required.
• Must hold a current DE LPN license, or compact state license.
