Job Description
We are looking for an experienced Home Care Licensed Practical Nurse (LPN) to join our dedicated team at ChristianaCare. As part of our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, supporting New Castle County, DE. This role offers flexibility with options for per diem, part-time, and full-time positions.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in coordination with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
