Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. The successful candidates will work in New Castle County, DE, with day or night shifts, one weekend a month, and 2 holidays a year.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must have a current DE LPN license or compact state license.
Job Types

Pay
$22.00 - $28.00 per hour
Expected Hours
20 – 40 per week
