Job Description
We are seeking dedicated and experienced Home Care LPNs to join our team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning. This position offers the opportunity to work full-time, part-time, or per diem, with competitive pay rates based on experience.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
