Job Description

We are looking for a dedicated Home Care Licensed Practical Nurse (LPN) to join our team at ChristianaCare. As an LPN, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients.
• Implement patient-centered care plans.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED.
• Current DE LPN license or compact state license.

Job Types

• Per diem, Part-time, Full-time

Salary

Pay: $22.00 - $28.00 per hour

Expected hours: 20 – 40 per week
