Job Description
ChristianaCare HomeHealth is looking for skilled Licensed Practical Nurses (LPNs) to join their team. As a Home Care LPN, you will provide quality care to geriatric patients in the community, develop care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.

Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
