Job Description
We are looking for an experienced Home Care Licensed Practical Nurse (LPN) to join our team at ChristianaCare. As part of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
- Experience in home healthcare is a plus.
