Job Description
We are looking for an experienced Home Care LPN to join our ChristianaCare HomeHealth team. You will be responsible for providing skilled nursing care to geriatric patients in the community, implementing care plans, and assisting in discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High school diploma or GED.
- Current DE LPN license or compact state license.
