Job Description
We are currently seeking experienced Home Care LPNs to join our team at ChristianaCare. As part of our HomeHealth division, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in collaboration with the healthcare team.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
