Job Description
We are seeking dedicated Home Care LPNs to join the ChristianaCare HomeHealth team. As an integral part of our organization, you will provide skilled nursing care and treatment to geriatric patients in the community, supporting New Castle County, DE. You will work closely with our patients, implementing care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
