Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a valuable member of the HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community while implementing patient-centered care plans. This role offers the opportunity to work full-time, part-time, or per diem, with a competitive hourly pay rate.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
