Job Description
We are looking for experienced Home Care LPNs to join the ChristianaCare team. As a Home Care LPN, you will provide skilled nursing care and prescribed treatments to geriatric patients in the community. Your role will involve implementing care plans, participating in discharge planning, and ensuring patient-centered care.
Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented and patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
