Job Description
We are looking for an experienced Home Care LPN to join our dynamic team at ChristianaCare HomeHealth. In this role, you will provide skilled nursing care to geriatric patients in the community, implementing care plans that are goal-oriented and patient-centered. You will work collaboratively with our supportive leadership team to ensure the well-being of our patients.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
