We are seeking experienced Home Care Licensed Practical Nurses (LPNs) to join our dedicated team at ChristianaCare. As a member of the HomeHealth division, you will have the opportunity to provide skilled nursing care to geriatric patients in the community. Join a supportive team with state-of-the-art technology and comprehensive training in OASIS-E.

Responsibilities

• Give skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement patient-centered care plans consistent with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED required.
• Must possess a current DE LPN license or compact state license.
