Job Description
We are looking for compassionate and experienced Home Care LPNs to join our team at ChristianaCare HomeHealth. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and ensuring patient well-being.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
