Job Description

Genesis Health is looking for a dedicated and licensed Practical Nurse (LPN) to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper nursing center reimbursement.

Position Highlights

• Direct the RAI process, encode MDS data, and transmit electronically.
• Develop individualized Care Plans based on resident needs and regulatory requirements.
• Provide training on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in healthcare settings.
• Familiarity with nursing practices and regulations.
• Proficiency in Windows applications; AccuMed experience a plus.
• BLS/CPR certification.
