Job Description

We are looking for a dedicated and qualified Licensed Practical Nurse (LPN) to join Genesis Health, a leading healthcare provider offering a range of services from short to long-term care. As an LPN at Genesis Health, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process, ensuring compliance with guidelines for MDS completion and transmission.
• Participate in creating individualized resident Care Plans in adherence to Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare facilities.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills, including familiarity with Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Current BLS/CPR certification required.
