Are you a compassionate and dedicated Licensed Practical Nurse looking to make a difference in healthcare? Genesis HealthCare is seeking an LPN MDS Coordinator to join our team. As part of our commitment to high-quality care and service, you will play a vital role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs), ensuring optimal patient care and center reimbursement.
Position Highlights
- Direct and oversee the interdisciplinary RAI process for accurate completion and transmission of MDS.
- Participate in developing personalized resident Care Plans in compliance with regulations.
- Provide training on clinical content and electronic MDS completion to new team members.
Requirements
- Active LPN license and good standing.
- Prior experience in healthcare settings, knowledge of nursing practices, and relevant regulations.
- Proficiency in computer applications, with AccuMed experience preferred.
- Demonstrated expertise in MDS completion and RAI/PPS processes.
- Current BLS/CPR certification.
