Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community. Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
