Job Description
ChristianaCare HomeHealth is seeking dedicated Licensed Practical Nurses (LPNs) to provide skilled nursing care and treatment to geriatric patients in the community. As part of our team, you will play a crucial role in implementing patient-centered care plans and ensuring the well-being of our patients. Join us in New Castle County, DE, on a full-time, part-time, or per diem basis.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
