Job Description
ChristianaCare HomeHealth is seeking dedicated Licensed Practical Nurses (LPNs) to join our team. As an LPN in home care, you will provide skilled nursing care and treatment to geriatric patients in the community, ensuring goal-oriented care plans are implemented effectively.
Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
