Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join our HomeHealth team at ChristianaCare. In this role, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Contribute to developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
