Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN in home care, you will provide skilled nursing care and treatment to geriatric patients in the community, implementing patient-centered care plans with a focus on discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in discharge planning.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
