Job Description
We are currently seeking experienced Licensed Practical Nurses (LPNs) to join our team at ChristianaCare's HomeHealth division. As an LPN, you will play a crucial role in providing skilled nursing care to geriatric patients in the community, implementing care plans, and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in accordance with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must hold a current Delaware LPN license or compact state license.
