Licensed Practical Nurse MDS Coordinator

Job Description

We are looking for a Licensed Practical Nurse MDS Coordinator to join Genesis Health, a leading provider of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in healthcare facilities.
• Proficient in general nursing practices and MDS processes.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
