Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN/LVN) to join Aveanna Healthcare. As an LPN/LVN, you will be responsible for providing skilled nursing care to patients, ensuring adherence to the care plan and physician's orders while upholding professional standards and confidentiality.

Essential Job Functions

  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Observe and assess patient condition, reporting changes to supervisors and physicians.
  • Document all activities, assessments, and coordination of care promptly.
  • Implement nursing care plans and take necessary actions based on assessments.
  • Recognize and address changes in patient needs to prevent risks.
  • Comply with Aveanna policies, state regulations, and infection control measures.
  • Educate patients and families on self-care techniques and disease prevention.
  • Maintain competencies through educational programs and skill evaluations.

Requirements

  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse.
  • CPR certification.
  • Proficiency in clinical assessments, documentation, and compliance.
  • Additional state-specific requirements may apply.

Preferences

  • Six months of recent clinical experience as a Licensed Nurse.
  • Home health experience is a plus.

  • Good organization and communication skills.

Physical Requirements

  • Must be able to speak, read, write, and understand English.
  • Ability to travel and lift up to 50 pounds.
