Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Execute goal-oriented, patient-centered care plans in alignment with available resources.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
