Job Description
We are looking for a dedicated LPN Home Care nurse to join our team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, following care plans and ensuring patient well-being. Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented care plans in line with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
