LPN Home Care - $22-$28 / hr

Job Description

We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN in our HomeHealth program, you will provide skilled nursing care to geriatric patients in the community. You will collaborate with our team to implement patient-centered care plans and assist in developing discharge plans. This role offers the opportunity to work full-time, part-time, or per diem hours, with compensation ranging from $22 to $28 per hour, depending on experience.

Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement care plans that are patient-centered and consistent with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Must possess a current DE LPN license or compact state license.
