Genesis HealthCare, a leading provider of healthcare services, is seeking a detail-oriented LPN MDS Coordinator to join our team. In this role, you will participate in the Resident Assessment Process through the completion of pertinent items on the Minimum Data Set (MDS) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items.

Requirements

• LPN license in good standing.
• Prior experience in a healthcare facility.
• Knowledgeable of nursing practices, regulations, and MDS processes.
• Computer skills including windows applications.
• AccuMed experience preferred.
