Job Description
Join our team at ChristianaCare HomeHealth as a Licensed Practical Nurse (LPN) and be part of our state-of-the-art Electronic Health Record system with comprehensive OASIS-E training. We are looking for experienced LPNs to provide skilled nursing care to geriatric patients in the community while working collaboratively with our supportive leadership team.
Responsibilities
- Administer skilled nursing care and prescribed treatments to geriatric patients.
- Develop and implement patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
