Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and collaborate on discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Collaborate on discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
Job Types: Per diem, Part-time, Full-time
Pay: $22.00 - $28.00 per hour
