In this role, you will play a vital part in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in accordance with guidelines.
• Contribute to the development of personalized resident Care Plans, addressing resident needs and expected outcomes.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital or long-term care settings.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer skills, specifically in Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification required.
