Job Description

Genesis Health is looking for a dedicated and skilled LPN MDS Coordinator to join our team. As a pivotal member of our healthcare facility, you will be responsible for participating in the Resident Assessment Process by completing essential items on the MDS and Care Area Assessments to ensure optimal patient care and facility reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process following guidelines for completion and transmission of the MDS.
• Contribute to the development of individualized resident Care Plans, addressing resident needs and anticipated outcomes in compliance with regulations.
• Provide training on clinical content and electronic completion of MDS items to new interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare facilities.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications; AccuMed experience preferred.
• Experience as an MDS Coordinator and in RAI/PPS processes.
• Current BLS/CPR certificate.
