As an LPN MDS Coordinator, you will play a key role in the Resident Assessment Process by completing MDS and Care Area Assessments, ensuring quality patient care and proper reimbursement.

Responsibilities

• Direct the RAI process, ensuring accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to interdisciplinary team members.

Requirements

• LPN in good standing with current state license.
• Prior experience in a healthcare facility, with knowledge of nursing practices and regulations.
• Computer proficiency, especially in Windows applications; AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
• Proficiency in RAI/PPS processes and maintaining BLS/CPR certification.
