Job Description
The University of Vermont Health Network is seeking a meticulous and experienced MDS Coordinator to join our team. Reporting to the Director of Nursing Services, you will be responsible for conducting comprehensive assessments of residents' needs and developing individualized care plans. The role involves close collaboration with Nursing Unit Managers and other healthcare team members to ensure residents receive the highest quality care. As an MDS Coordinator, you must adhere to the Vermont Nurse Practice Act, Nursing Standards of Practice, and all relevant departmental policies.
Qualifications
- Graduation from an accredited Nursing Education Program with current RN licensure or compact licensure recognized by the State of Vermont.
- Minimum 5 years of MDS experience post-2019.
- Three to five years of experience in a long-term care environment as a Team Leader or Charge Nurse.
