Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a Licensed Practical Nurse (LPN) to join our team as a Minimal Data Set Coordinator. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Oversee the interdisciplinary RAI process in compliance with guidelines for MDS completion and transmission.
• Contribute to the development of personalized resident Care Plans, meeting Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare facilities.
• Familiarity with nursing practices, regulations, and MDS completion.
• Proficiency in computer applications including Windows; AccuMed experience is a plus.
• Previous MDS Coordinator experience preferred.
• BLS/CPR certification required.
