Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis HealthCare is looking for a dedicated Licensed Practical Nurse to join our team as a Minimal Data Set Coordinator. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Responsibilities

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with completion guidelines and electronic transmission standards.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in a hospital, long-term care facility, or similar healthcare setting.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in computer skills, including Windows applications.

Preferences

• Experience as an MDS Coordinator.
• Demonstrated proficiency in RAI/PPS processes.
• AccuMed experience is a plus.
