Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a Licensed Practical Nurse to join our team as a Minimal Data Set Coordinator. In this role, you will play a crucial part in the Resident Assessment Process by completing MDS items and Care Area Assessments, ensuring proper patient care and reimbursement.

Position Highlights

• Direct and oversee the RAI process, including MDS completion, encoding, and transmission.
• Develop individualized Care Plans meeting Federal and State regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Current LPN license in good standing.
• Prior experience in hospital or long-term care settings.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in MDS and Care Plan completion.
• Knowledge of RAI/PPS processes.
• Computer skills, including AccuMed experience.
