Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a diligent and detail-oriented Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team at Genesis Health. In this role, you will play a crucial part in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process to ensure accurate completion and electronic transmission of MDS.
• Contribute to the development of personalized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Licensed Practical Nurse in good standing with current state licensure.
• Prior experience in hospital, long-term care, or related healthcare setting.
• Familiarity with general, rehabilitative, and restorative nursing practices and regulations.
• Proficiency with windows applications; AccuMed experience preferred.
• Prior experience as an MDS Coordinator with demonstrated expertise in RAI/PPS processes.
• Current BLS/CPR certification required.
