Job Description

We are looking for a dedicated Minimal Data Set Coordinator - Licensed Practical Nurse to join our team at Genesis Health. The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process following completion guidelines and electronic transmission of the MDS.
• Participate in developing personalized resident Care Plans based on individual needs and expected outcomes.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare setting, such as hospitals or long-term care facilities.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications, with preferred experience in AccuMed.
• Prior experience as an MDS Coordinator and demonstrated expertise in RAI/PPS processes.
• Current BLS/CPR certification.
