Genesis Health is looking for a dedicated Minimal Data Set Coordinator - LPN to join our team. As the LPN MDS Coordinator, you will play a key role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement. Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process, adhering to completion guidelines and electronic transmission protocols.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and expected outcomes in compliance with federal and state regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital or long-term care settings.
• Proficiency in general nursing practices, medical procedures, and long-term care regulations.
• Computer literacy with Windows applications.
• AccuMed experience preferred.
