Responsibilities

• Perform home visit Physical Therapy assessments and deliver patient care.
• Utilize EMR documentation systems for accurate record-keeping.
• Collaborate with patients to assess medical conditions and rehabilitation potential.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• CPR Certification.
• Familiarity with Hello Note EMR system is a plus.
