Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct thorough home visit assessments for patients, evaluating medical conditions and rehabilitation potential.
• Proficiency in EMR documentation; experience with Hello Note is a plus.
• CPR Certification is required.
