Responsibilities
- Conduct home visit Physical Therapy assessments and provide direct patient care.
- Assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
- Proficient in EMR documentation.
- Maintain CPR Certification.
Requirements
- Graduated from an accredited Physical Therapy (PT) program.
- Unencumbered Physical Therapy (PT) license for New York.
- Experience in conducting home visits and assessments.
- Familiarity with EMR documentation.
