Responsibilities

• Administer skilled nursing care and prescribed treatments to geriatric patients at home.
• Develop and implement patient-centered care plans in alignment with available resources.
• Ensure IV proficiency and provide IV functions as needed.
• Participate in discharge planning for patients.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of employment.
• Active DE RN license or compact state license.
• Minimum 2 years of recent HomeHealth RN experience.
• Prior experience in Home Healthcare and Med-Surgical settings preferred.
