Job Description
We are looking for an experienced Home Health nurse to join our team at ChristianaCare. As a Registered Nurse (RN) Field Nurse for Home Care, you will provide skilled nursing care to geriatric patients in the community. You will be responsible for developing and implementing patient-centered care plans, ensuring goal-oriented treatment, and facilitating patient discharge when necessary.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement goal-oriented and patient-centered care plans.
- Plan for patient discharge when medically appropriate.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- 2 years of recent Home Health RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
- IV proficiency required or willingness to train.
Additional Information
- Job Types: Permanent, Part-time, Full-time
- Expected hours: 20 – 40 per week
- Pay: $32.00 - $44.00 per hour
