Job Description
We are looking for a dedicated and experienced Registered Nurse (RN) Field Nurse to join our HomeHealth team at ChristianaCare. As an RN, you will provide skilled nursing care and treatment to geriatric patients in the community, develop patient-centered care plans, and ensure the successful discharge of patients.
Responsibilities
- Administer skilled nursing care and prescribed treatments to geriatric patients in the community.
- Develop and implement goal-oriented, patient-centered care plans.
- Create discharge plans for patients.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of hire.
- Current DE RN license or compact state license.
- Minimum 2 years of HomeHealth RN experience within the last 12 months.
- IV proficiency preferred or willingness to train.
